Practice Training Afternoon

The practice will be closed for calls and unscheduled visits for the whole afternoon (from 1.30 – 6.30) on the following date:

Please note there is a 48hr response time.

Patient Access

Using Patient Access on your computer or mobile, you or a proxy (family member or carer) can: –

• order repeat prescriptions
• book and cancel appointments
• view your medical record

To get started:

• Download the online registration form by clicking here
• Send us proof of your ID, by emailing a photo of your head shot alongside your passport or driving licence, to SMCCG.MaplesMedical@nhs.net, and we will contact you via email or text.
